  • Authorise Aviva Direct Ireland/Aviva Insurance to communicate with your trusted contact person.
  • Understand that this does not authorise your trusted contact to access or transact on your behalf.
  • Understand that this trusted contact person is optional and you may withdraw it at any time.
  • Understand that you may change your trusted contact person at any time by completing a new form.
